· Active Duty Service Members:  Leave & Earnings Statement (LES), Active duty Enlisted Record Brief (ERB)/Officer Record Brief (ORB), Enlistment Contract

· Veterans: DD214 (Discharge Details Form 214), NGB form 22 (National Guard Bureau similar to a DD214), Veterans Health Identification Card (VHIC), or Veteran Identification Card (VIC), Driver's License or DMV-Issued State ID with VETERAN endorsement

· Retirees: Retiree Account Statement (eRAS), DD214 or NGB form 22 (must show 20 years of service or the reason for discharge was retirement)

· Surviving Spouses & Household Members: upload any of the military documents listed in the categories above. Surviving Spouses can also submit a Casualty Report (DD Form 1300)
